Catalyst 4000 AGM failing to boot

i installed an AGM on a 4507 switch. i then did some VOIP configurations on the AGM through its console port and not via the supervisor engine. the problem i'm facing now is that the AGM fails to boot and gives the following message

open: file "c4gwy-io3sx3-mz.122-13.T5.bin" not found

open(): Open Error = -1

loadprog: error - on file open

boot: cannot load "flash:c4gwy-io3sx3-mz.122-13.T5.bin"

How do I restore the AGMs factory boot settings?

Does configuring he AGM through its console disturb the AGMs boot registers and make it go offline?
