i have 15 catalyst 4006 switches.All are connected to Catalyst 6509.
And on all my catalyst 4006, IP phones are connected.
I have certain MAC Addresses of IP phones, i dont dont know on which module/port they are connetected.
how is it possible to find out from MAC address of IP phone on which Module.port its connected.
I dont have Call Manager.

You can connect to the IP phones using a web browser :-
Got to device information section and the mac address should be displayed. For more info see :-

All phones run CDP. Your switch should definitely be running CDP. Give a show cdp neighbor or show cdp neighbor detail to get the mac-address and IP address of IP phone. This command will also show the port on the switch where each individual phone is connected. Hope that helps!
