MSFC2 on slot 1 of Cat 6509 unreachable

We have a Cat 6509 with two Sup2 modules plus MSFC2's. We upgraded the Native Switch software and MSFC software on both modules. Before upgrade, we could reach both MSFCs. After system reset, Module 15 was shown as not installed. Active supervisor was in slot 1, and we used the 'switch console' command to reach module 15 to see if it was in rommon mode. But only 'type ^C^C^C to switch back' message was displayed and we did not get any cli prompt from module 15. When we 'switch supervisor' to slot 2, 'switch console' command works fine to reach module 16. Does anyone have a solution for this problem?
