I get this bounced back in some, but not all emails when my IOS firewall is up, but it will send if the firewall is disabled. Obviously this is not good, because I need the firewall up at all times. The router is a SOHO 91 with IOS 12.3(2).

Figured out my own problem. I removed the smtp inpection due to the fact that my mail server uses esmtp.
form global config mode:
no ip inspect name myfw smtp
