With the CISCO IOS Content Filtering enabled. What reporting and logging options are there and what is the level of detail?

You can "debug ip urlfilter ..." to see some feature debugs.

but also under the class in the policy map you can use the log keyword. The router will then generate a syslog for every url that matches that class in the policy and the action is enforced.
