3560 poe switch stuck in loop

Hello

I recently reset a 3560 poe i have reset the switch but i keep getting:

nameofswitch-mdf1 con0 is now availble

press RETURN to get started.

Then I get

User Access  Verification.

Password:

'Course I don't know the password, which is i reset it.

Xmodem file system is available.
The password-recovery mechanism is enabled.

flash filesystem.  The following commands will initialize
the flash filesystem, and finish loading the operating
system software:

    flash_init
    boot

I tried several attempts. If I choose boot I get stuck in the loop. I do get the switch: prompt . But that's all. Never saves the flash_init. Get the same message everytime I boot.

Jezz the one thing I could always do, if nothing else; is reset the switch!

Couple of things I tried

If I try enable I get

unknown cmd: enable.

Copy start run 

start: no such device.

This switch may have been in a stack. Would that make difference?

So now I am thinking it's the IOS image.

Figuring its a image issue
I am trying to recover the Cisco IOS Image

switch: format flash:

switch: set BAUD 115200

close putty and reset baud rate

switch: copy xmodem: flash:c3560-ipservicesk9-mz.122-53.SE.bin

Now i getting:  xmodem: : I/O error
Tried a slower baud rate nada. Guess my next step is to setup a TFTP server
and try that.
