It maybe a baud rate issue. Try using different baud rates using Rommon and if that does not help, RMA the switch.

Hi there
After turning power on, at the end of 3-4 minutes, which LEDs are on? Please use your cell phone and share with the photo.
The screenshot you show indicates possible problems in connecting to console. For 2960X, you can use either RJ/45 to DB9 serial connection OR connect via USB console. What type of hw are you using to connect to the console? Did you try using the same setup with any other Cisco device, like a router for example?
Kind regards .... Palani

It is not related to the console cable because i used on other devices (switches, routers) and it works. I am connecting via RJ45 to usb console cable..

If the same console cable (with the correct settings) work with other router/switches, then we can safely conclude something wrong with the the switch-side. It is possible that the baud rate setting was messed up within ROMMON. This was mentioned by Reza also.
Are you able to send the break/hw signal to the switch?
This model switch allows for console connection via USB. This is documented here
Give it a shot and share what your observations are.
Finally, I concur with Reza that you may be looking at hw replacement.

Based on your (earlier) responses, we can safely conclude the console cable is working fine. Did you try to connect via USB console? Is the switch functioning ok otherwise?
Kind regards ... Palani
