This is hard to say for sure, I believe that this is happening as part of the call dropping, meaning the agent and customer are in mid-conversation call drops and that shows up on the screen.
Thank you all for helping out on this issue. Ended up being something on my laptop was blocking the TFTP so I was able to get the image loaded using a different laptop without a corporate image on it.
