I have no idea about the Polycom unit, but you mention H.323 in your question. Are you by any change using CUCM 10.5?!
There is an known bug that disables H.323 ones in a while. i have a script running that log into every Conference unit ...
After a long search i've found the unlikely solution...
Somehow his own account was added in his contact list, with the name of the first contact of his list.
removing the account did the trick.
