Did you intend to do certificate authentication along with username and password?If not, and the association is pending a certificate authentication, then you have the wrong EAP type selected.
It looks like the LWAPP discovery is coming in on the wrong network. Could the switch port for that AP be configured with the wrong VLAN?Hope this helps.
