11150 Switch stops transmitting at random

I have consistent problems with our 11150 switches, one will stop transmitting like it is the "backup" switch but it never transitions from being "master" so all traffic is stopped. It never shows a link failure or a state transition. There are no errors or drops on any of the next hop switches. We have replaced the hardware and still have the problem. Here is the error in the debug log:

MAR 3 22:52:27 5/1 425 FLOWMGR-7: Resetting port 2c01f00 ending at 114641

MAR 3 22:55:12 5/1 426 IPV4-7: Ipv4SfmForwRx: Ingress circuit non-exist

what is the current software version ?

Could you capture a 'flow stat' from debug mode and send it to us.

Please standby on this as I see you have a TAC case open. We are investigating this with regards to EPIF resets on the CSS. The engineer you are working with was instructed to get a "flow statistics" in debug mode.
