SFP (GLC-LH-SM) is not working in Cisco ISR 4431 Router

Hi,

I am using cisco ISR 4431 router and  I am trying to install sfp (GLC-LH-SM) but it is not working, as  I checked on the log  there is an error showing up " transreciver-init-failed "

Router IOS is IOS XE 16.6.4

Please help me to resolve this.

It means the transceiver is dead. 

What happens if the transceiver is replaced?

Can you you post the picture of the SFP - is this Cisco SFP ? or unbranded ?

I have upgraded the ios 16.6.4 to 16.9.5 but still is persisting.
