ROM-MON upgrade fails

When attempting to upgrade rom-mon on 4451X from 15.4(3r)S to 16.7(4r) on IOS v15.6(1)S4 I get "Error connecting to command relay server" and "%SMANRP-3-CMDCONNECT: Connection to local interactive relay server failed: connection timed out; remote host not responding".

The command I'm using is 'upgrade rom-monitor filename bootflash:isr4400_rommon_167_4r_SPA.pkg all'

I've done this on about a dozen other systems without issue. Anyone see this or know what I can do to correct?

Was there a resolution for this issue?  We are having the same exact issue with the rom-mon update but not on all routers.
