D - creates monitor call on A -> B -- Exception
How can i create two monitor calls and associate them both with one Call?
One monitor call works fine, but if I try to create another one have "Monitor request failed" Exception.

A device can support only one monitoring session. The second monitor session will fail. In your case the options are:
1. If both A and B are observed start the first monior session on A and the second session on B. Both A and B should be phones that support monitoring feature.
2. Instead of a monitor session from B, consult from C to D and complete the conference.
