The main part of the URL is defined in the Enterprise parameters page as well as on the device page. I tried http://<cucm>:8080/ccmcip/getservicesmenu.jsp?device=<device-name> and http://<cucm>:8080/ccmcip/getservicesmenu.jsp?deviceName=<device-name>. Each just gave me the enclosing CiscoIPPhoneMenu element as if the device had no services deployed.
