CRS Update Utility error, can't connect to LDAP

when try to use CRS update utility, got error:

The Update Utility is unable to connect to the lDAP. please check you connection to the LDAP and return the update Utility.

LDAP ip address is correct, service is ruing.

I'm having the same issue trying to upgrade from 4.0(4) to 4.0(4) SR1.

I've reinstalled CRS from scratch and still can't upgrade to SR1.
