we have 4 phones with the same number on the second button, and after 2 phones are active on the line, the 3rd call that comes in goes to voicemail yet there are 2 other phones that can answer if presented. is this a system limitation.

It's hard to tell you if this is a system limitation or not if you do not mention what system you're using (CUCM, CME, CUCMBE, UC520) and the version.

If i were in your situation i would check 'Maximum Number of calls' and 'Busy trigger' in the DN configuration page. Since you have 4 phones with same extension you can set them as Maximum Number of calls - 5
