** Possible Resolution **
1. On the ip phone, you can select the softkey 'cfwdall'
2. You can go to CCMAdmin and on the phone configurations, you can remove/Add the call forward options if any, and click on update.
3. You can also go the CCM User page, and login as a user. Select the associated device and edit the call forward option.
