CME 12.1 + 8800 SIP - Expected functionality of cbarge?

I am having really strange results trying to test cbarge with CME 12.1 and 8800 phones. Some of what I am seeing 

All phones have appearance of 101

Individual extensions

Phone A 401

Phone B 402

Phone C 403

plus there 2 FXO ports connect to another PBX

1. Place call from DN101 on phA out via FXO1  = Barge DN101 on phB successful

2. Receive call from FXO1 on phA DN101 = Barge DN101 from phB fails with fast busy

3. Receive call from FXO1 on phA, Confer DN402 into 101 call on phA, phC barge button changes from green background to greyed out - barge appears to be have been disable as if privacy enabled, returns to normal green status after dropping conference w/ DN402 (still fails to barge as scenario #2). 

4. Place call from DN101 on phA out via FXO1 , Conference 2nd call by dialing out via FX02 on phA = Barge is lit greem on phB but fails with fast busy when pressed. 

Some serious madness here. Anyone care to comment? yes have a TAC case open but haven't spoken to engineer in depth about this yet.

Are you hitting barge or cbarge softkeys?
Are you limiting max lines per DN?
What codec being used during the call and is that codec add in CFB? show
call activ vo com
Have you checked if your CFB registered to CME before barging? show sccp

Relevant key template info - cBarge is configured

Are you limiting max lines per DN?

Have this configured for the shared DN
"shared-line max-calls 8"

On the Voice reg pool
