We configured LDAP at our CUCM 6.13 cluster.
However, the corporate directory is not shown as an option under the directories button.
The "url directories" under enterprise parameters has the following:
http://cucm-ip:8080/ccmcip/xmldirectoryinput.jsp
What are the correct setup steps to configure "corporate directory" lookup on phones? What documentation should we follow?

I don't think the URL is correct.
The correct one should be like this:
http://192.168.1.10:8080/ccmcip/xmldirectory.jsp
Where 192.168.1.100 is the IP address of CUCM. Yours is 'xmldirectoryinput.jsp' instead of 'xmldirectory.jsp'.

Which document should I look into to get more information about personal/corporate directories setup?
We never used personal directory before, so we would like to learn more about the setup detials (pros/cons).
One of things that we modified on CCm4.0 was the corporate directory displayed name-search priority (e.g. last name first, first name second and number last). We also want to find out how we can modify this section.

CCM 4.x is an open box. You can make changes to the xml file on CCM 4.x and customize the directory.
CCM 5.0 and above are 'appliance' (ie. locked down). You cannot modify the XML file any more. If you want to customize the directory, you'll have to host the XML file on a web server (e.g. Microsoft IIS). Then configure the 'URL Directory' enterprise parameter to point to the customized XML.

We created an app. with the SDK for CCM4.0 to allow AD look-up for corporate directory without integrating with AD. It is still usable with the CUCM6.13.
However, when we use the URL for this app. in "URL directory" section under "enterprise parameters" we do not get the personal directory option (#4) on the phone.
Is there any other way to utilize our existing app. and still have the option #4 available to the phone?
If we can do that, we would not have to generate another XML file as you suggested.

You have to modify the existing XML to include Personal Directory. Your developer should be able to do that.
