Cisco ISE 2.4 Guest Portal - International Phone Number

I really need some guidance to fulfill end customer request.

Is it possible to use custom fields and create a dropdown box to force guest to pick the phone number international country code or fill it down. Can I place a custom field before Phone number field?

I can't seem to get any script in "Optional Content 2" to work. I toggle to HTML, paist, then toggle out and save. Nothing changes in the interface.
