I've had this issue before with a policy set with 200+ authz rules. It happened when I edited an authz near the top, then scrolled to the bottom to save, but in the mean time I didn't give the page time to complete loading the newly displayed authorization rules I scrolled past. Maybe it will help you, if not then I agree with Greg.

I've got the same problem on ISE 3.3 patch 4, and this solution did not work initially. It seemed to work if I hit the "save" button at the top of the page rather than attempting to scroll to the bottom of the authz rules.

This works for me. Hit into this bug and it seems only the top 'Save" button works.
