Catalyst 2960-X series booting loop

Cisco 2960-x switch boots repeatedly after a power failure.
The log below is what I find suspicious.
Please help me.
-Current IOS version : 15.0(2)EX5
 

FIPS: Flash Key Check : Begin
FIPS: Flash Key Check : End, Not Found, FIPS Mode Not Enabled

POST: MA BIST : Begin***ERR: S88G MDIO Read - Address is not decoded.

POST: MA BIST : End, Status Failed

POST: BIST failed
POST: ACT2 Authentication : Begin
POST: ACT2 Authentication : End, Status Passed
driver class subsystem initialization failed

Hi There!

I would recommend for you to install the Cisco Recommended IOS, since yours is quite a bit old.

Install it via xmodem, if you change the baud rate to 115200 it wont take that long.

If the issue persists it could be a possible hardware issue.

Hardware is dead.  Contact Cisco TAC and arrange for an RMA.
