For some reason the Stored Device Credentials stopped working, authentication failed. However the same User/Pass works with the Application Update piece and www.cisco.com. Any reason why this could happen? I know I am putting the correct CCO User/Pass as I have tried many times. In between these times I made sure the account was not locked out by logging into the cisco.com site. This was working before and just stopped working sometime after 4/11/2012. I am running version 5.7.5 and even upgraded to 5.7.6 to see if that would fix it. i am using a Windows XP SP3 machine.

I am seeing the same thing. I haven't had time to open a TAC case but I suspect it's another artifact of Cisco's continued updating of their web site. That same thing was confirmed to have broken LMS online updates a short while back.
