Data from stack_data_connection.jpg & stack_power_connection.jpg are useless.
show_switch_snapshot.jpg is showing the stack were all powered up simultaneously. Have you tried checking the power for switch stack #5?

The 3750-X models have a data stack and power stack. The power stack is limited to four switches, so a fifth one would have to be in a separate power stack.
You can form a data stack however with up to 9 members. When you plug into console on switch #5, what does it show for "show switch detail".

Your switch is not booting IOS and is is the pre-boot condition known as "rommon". (Read Only Memory MONitor)
This is usually due to a deleted or corrupted IOS image. You need to recover the image and boot the IOS fully for the switch to be able to join the data stack. Here is a reference to the commands available to you in rommon mode.
Use dir to figure out what you have and then copy to move the files into flash (if necessary). Then boot using the restored image.
