Cisco 4431 router dual power supplies

I have a Cisco 4431 router with dual power supplies. The main power supply at times will fail and it's not switching to the 2nd power supply. I have confirmed we are not losing commercial power. The LED's on the back of he router show both power supplies to be healthy. Any help is appreciated.

 - If the LED's show power supply healthy , how do you then conclude a possible intermediate power failure ?

Based on the error logs. It could totally be something else. I’ll post the IOS version should have done that initially.
