I consistently get the following errors when attempting to perform a software upgrade via Prime:
SCP
Flash Validation successfully completed.Loading Image File to Device : Copying image cat3k_caa-universalk9.16.12.06.SPA.bin to the flash flash: failed. Error occured while communicating with the device. Check device credentials and SSH/telnet reachability..
FTP
Flash Validation successfully completed.Loading Image File to Device : Copying image cat3k_caa-universalk9.16.12.06.SPA.bin to the flash flash: failed. Error occured while executing the command copy ftp://ftp-user:LD7SIZKcSLGK7BX@172.26.16.170/cat3k_caa-universalk9.16.12.06.SPA.bin flash.Command Output : ]? cat3k_caa-universalk9.16.12.06.SPA.binAccessing ftp://*:*@172.26.16.170/cat3k_caa-universalk9.16.12.06.SPA.bin...%Error opening ftp://*:*@172.26.16.170/cat3k_caa-universalk9.16.12.06.SPA.bin (Timed out).
I am able to connect via SSH to the switch stack consisting of two WS-C3650-48PS. I attempted the process and it continued to fail when I removed:
snmp-server view ViewName ciscoFlashMIB excluded

Is the target switch a standalone or a stack?
Go to the target switch and post the complete output to the command "dir".

- Anything in the logs of the switch when Prime is doing this ? Also check https://www.cisco.com/c/dam/en/us/td/docs/net_mgmt/prime/infrastructure/3-9/supported/devices/PI39-Supported-Device-List.xlsx , verify your device is supported in the current Prime version that you are using , also look at the minimum IOS requirement(s)

Nothing useful in the logs.

- Remove a particular device from Prime (which has the problem). Enter it again , but press Verify Credentials first.

I did that prior to making the original post to include verifying credentials. Credentials register as 'success'. Unfortunately, I received the same failed results after removing, re-adding, and verifying credentials.

The note under Distribute a New Software Image to Devices states:
To ensure that there are no SNMP views blocking access to the CISCO-FLASH-MIB, remove the following command from the configuration for all routers and switches (if present) on which you want to download a software image:
The software image distribution and image import may fail with authentication error, if you use special characters in the protocol password.
Right below the note shows: snmp-server view ViewName ciscoFlashMIB excluded
I issued: no snmp-server view ViewName ciscoFlashMIB exclueded
