Determining Factor to identify the reason for URL Reputation Change

URL Reputation for one my major site got changed to Questionable.

As we Block websites with “Questionable” reputation, this site got blocked in our firewall.

After 2 days I see from talos it is changed to "Neutral"

What are the catalyst changes it ? How can I prevent not to be changed by Talos ? Frequently.
