The possible causes follow:
- The FTP or TFTP server is unreachable.

- The Cisco IOS Software image does not exist on the FTP or TFTP server, or the name has been mistyped.
- The Cisco IOS Software image on the FTP or TFTP server does not have appropriate access permissions.
Resolution
To resolve this issue, refer to the following options:
- Verify connectivity to the FTP or TFTP servers.
- In the case of the FTP server, verify the username and password.
- Verify the presence of the Cisco IOS Software image on the server.
- Verify access permissions of the Cisco IOS Software image on the server.
