I am trying to setup the familyshield dns. I have tried following the configuration instructions on both of these websites:

With my computer, I cannot get Opendns to work at all. I cleared both my browser cache and flushed my ip, then restarted my computer multiple times, and I still am not blocked form internetbadguys.com.
With my iPhone, everytime I change the DNS to the familyshield dns, it changes back automatically to the original.
I would really like to just set this up on my router, but since I have at&t u-verse, I'm not sure if I have any options to do that without another router. Any suggestions on this would be helpful too.

This is the DNS service you're using, clearly not OpenDNS FamilyShield. You must also configure DNSv6 resolvers addresses for FamilyShield if you want to use the IPv6 internet.

And yes, ISP supplied routers often do not allow to change DNS settings. You'd have to move to an OEM router.

So on top of the instructions I linked in my original post, I have to do what you've posted here - how do I go about configuring DNSv6?
Also, for my iPhone, how can I set this up? I am still having that issue of the DNS being reverted back to the original after changing it.

You enter the DNSv6 addresses into the IPv6 properties of your active network connection, in the same way you did for IPv4.
For iOS you can use an app like https://itunes.apple.com/us/app/dns-override-set-dns-for-wi-fi-and-cellular/id1060830093?mt=8

I tried entering the familyshield DNS 208.67.222.123 into the ipv6 properties and Windows says that that DNS is not a valid IP address.

I'm so sorry! I did read it, I just didn't know that those were DNS addresses. It's working on my computer! But it doesn't block inappropriate images on Google images...any way to do that?

A DNS service generally cannot block and does not know about images or other objects. It can just block domains where objects are hosted on. Google domains are not being blocked by FamilyShield. You could go for OpenDNS Home and block Google domains at your dashboard.
If this is not what you want, you will have to use the tools provided by the providers where you can block content more granular.

So OpenDNS Home has a different DNS and would have to be manually setup to block certain domains?
Also, my phone is on iOS 8 and I wasn't able to install the DNS override app. Are there any other apps or other workarounds?

It doesn't matter if you use the FamilyShield addresses or the OpenDNS Home addresses, you can use your dashboard by configuring a dashboard network at https://dashboard.opendns.com/settings/ where you can block additional stuff then, by category of domains or by individual domains.
Regarding an app, you may have a hard time still finding a related app for iOS 8. It may be that you can only use the instructions at https://support.opendns.com/hc/en-us/articles/228009167
Best would be if you could configure OpenDNS on a router or on an internal DNS server.
