I use OpenDNS Family Shield and all the appropriate sites thankfully seem blocked.
I added one site to be never blocked and I cannot unblock it. I clicked apply, waited a few minutes and flushed my cache but no luck.
My IP is up to date in Dashboard.
I'm open to ideas.

You cannot whitelist what is being blocked by the FamilyShield resolver addresses 208.67.222.123 and 208.67.220.123. That's totally normal and as intended.
You would have to use the normal OpenDNS resolver addresses and to block the categories Pornography, Tasteless and Proxy/Anonymizer (which has the same effect as FamilyShield), and then you can whitelist whatever you want.

Thanks. I missed that I guess.
I changed my DNS to 208.67.222.222 and 220.220 and when using Moderate filtering I still can access adult sites. According to welcome.opendns.com I have set up correctly.

Well, configuring 208.67.222.222 and 208.67.220.220 and selecting the domain categories Pornography, Nudity, Tasteless and Proxy/Anonymizer has the same effect like using the OpenDNS FamilyShield addresses. If this is not the case, then you have a problem of a different nature, like not using your dashboard settings consistently, because your IP address may not always be correctly registered at https://dashboard.opendns.com/settings/

nslookup -type=txt debug.opendns.com.
nslookup www.exampleadultsite.com.
