I have the 2960X Catalyst switches using the GLC-LH-SMD SFP Module. The facility existing cabling plant is OM3. Do I need to use the Mode conditioning fiber patch cables and if so where can I get such specifications based on existing infrastructure??

In researching your question, to my surprise it looks like mode conditioning patch cords are not required (or recommended) for OM3 fiber.

Cisco GLC-LH-SMD is complaint with 1000BASE-LX/LH standard that can operate both over MMF and SMF.
According to Cisco data sheet, this SFP, when used over legacy multimode fiber type like OM1 and OM2, the transmitter should be coupled through a mode conditioning patch cable. In your situation, it doesn't need it.
