I have ATA 186 "A" setup to forward calls from Phone1 to Phone2 in case Phone1 is busy/no answer; and to forward calls from Phone2 to another ATA 186 "B" in case Phone2 is busy/no answer.
Problem: in case Phone1 didn't answer, a call is forwarded to Phone2 but Phone1 is still ringing!

Sounds like there may be a software issue. Be sure you're running the latest ATA186 software (2.15).

I'm running the ATA186 software Version: v2.15.ms ata186 (Build 020723a)
