When a call is active on the line and a second call comes in on the 2nd channel of the same line, the caller-id is not displayed.

Is the problem occuring on ISDN link or H.323 link?

I would upgrade to 4.1.3sr3b before doing to much debugging.

This happens internally, only 4-digit extension is seen not the name, although phone configuration has the name in.

Which phone model and load version?
Please attach the detailed CCM trace.
