Some of our users are getting "Login Unsuccessful" error [6] when they try to login into either 7940 or 7960 phone. We checked the device assocation, login parameters, device profile, and default device logoff profile - all these appears to be configured correctly.

what is the authentication url in enterprise parameters? If it has dns name, change to the ip address of the ccm.

My problem was the know issue - EM service was running off of Subscriber CCM.
Bug ID: CSCec19768
As soon as i took, Subscriber IP reference from Active Directory - the problem went away.
