Due to lack of specific error messages, I suggest you simply retry the upgrade, or open a TAC case, as it may require log analysis to determine upgrade failure cause.

Did you run the HX health checker script prior to upgrading?

Yes, I restarted the process the upgrade had the same results. Yes, I pass the health checker script.
