7204VXR with NPE225 boot problem

We have a 7204VXR chassis with a NPE225 module together with a C7200-I/O-2FE/E port adaptor that has begun to fail to boot. I've pasted the console output that's displayed at boot below:

from reading several relevant Cisco documents I am beginning to feel we are lucky this router has ever booted:

The part numbers of our equipment is as follows:

2F/E port adaptor: 800-07114-04-A0 (sticker on outside of adaptor)

2F/E printed ciruit assembly: 73-5003-04-B0

NPE225 processor : 73-3453-04-A0 and 73-4043-02-B0

Thanks for any help any-one can provide...this equipment is not covered by warranty.

Not applicable

1 of 2 things could be causing this.

1. There is no/no valid bootflash image on the flash of the IO controller.

2. There is no FLASH SIMM installed in the IO controller.

You can check this by using "show bootflash". If it reveals nothing then your problem lies there. Otherwise try to perform a format and then copy an image there, failing that a physical inspection will be needed.
