Hi Jack,
Hope you're doing well. On the cat6500 switch, the CSM module needs minimum CatOS code 7.5(1). The recommended code is 7.6(9). This is mentioned in the release notes at the link below.
http://www.cisco.com/univercd/cc/td/doc/product/lan/cat6000/relnotes/ol_4498.htm#wp419671
Please note that by default the sup1A ships with 64MB DRAM. To upgrade to codes 7.5(1) and above, it is recommended to have minimum 128MB DRAM. Hence, it will be advisable to upgrade the DRAM before you upgrade the code.
Also, I found the following link.
http://www.cisco.com/univercd/cc/td/doc/product/lan/cat6000/relnotes/ol_6897.htm#wp16934
This link mentions that you must have a sup1A with a MSFC. From your show module, it looks like you dont have the MSFC daughter card. Hence, you wont be able to support the CSM module with this superviro module.
Hope that helps.
regards

This does look like hardware failure. It might be a good idea to test it on another slot if you have a spare slot.
