How does router know distinguish between incoming and outgoing traffic?

How does the router distinguish between incoming and outgoing traffic inorder to apply policy map?

This should be based on the Interface where you have applied Policy MAP.Ideally,interface policies take precedence over the global service policy.

Hello router only do the forwarding based on the information provided in the IP packet(source ip and destination ip) and compare it with the RIB and forward the traffic to which the routing table points and if policy is applied in the interface that is followed
