I have a 6009 Cat switch that, twice in 3 months, one of the 5 WS-X6348-RJ-45
modules has failed. By saying fail I mean all port status lights goes Orange. In both cases someone had "reseated" the card prior to me having a chance of checking the status in the IOS. Anyone have an idea what would cause this ? Is it possible for an entire module to put itself in Errdisable mode ? I realized the card could be failing but I wanted to rule out others causes before swapping HW.

What software version are you running? Make sure you are running latest 5.5(x) or 6.3(x) to avoid some bugs such as

Your switch is vulnerable to the bugs I mentioned in my previous post and also CSCdu03935. I suggest that you consider 5.5(15) which is our latest general deployment.
