Recently saw this message in the log of a Cisco 901
_soc_xgs3_mem_dma: FP_COUNTER_TABLE.ipipe0 failed(NAK)
Has anyone seen this before? What causes this and how do i go about resolving this?

It look like a hardware failure (it's DMA error during communication with a module). But sometime it may be just software issue. Or, issues like this one may be caused by excessive electromagnetic noise (comming either from mains or from a data cable).
Power cycle the switch. If it will not help, then consider it hardware failure.
