User Fields not showing

Has anyone seen a problem where the User fields do not appear to be populating correctly. I am importing via a CSV file and having problem with one server (have 3 on site) the server just had a crash of the hard drive and complete rebuild. I haven't run into this problem before so any assistance would be appreciated.

The main problem is that when I go to the add dynamic fields, there aren't there?
