cisco 4003 mod 1 testing failed

Hi，everybody

my cisco 4003 is crasy.today,i power off then reload it. the mod 1 is testing failed. the following is show test information.

And I try to boot tftp,also failed

rommon 74 >boot tftp://10.1.1.3/cat4000.6-3-5.bin
3:44.202135: Please set TftpServer variable to do tftp downloads

Hi im also having this problem,.

We are currently using WS-C2980G Series and Switching Supervisor also is failing
