ASR 9904 major alarm

ASR 9904 using DC V2 power supply its major alarm persist after turn on power supply , it’s disappear only after remove and re-insert the power module, can anyone know the cause?

Hello,

It could be a faulty unit or possibly a bad connection?

Do this happen every time you turn the power supply on?

Can you replace the power supply with a known good one to see if it works without having to re-insert? If the new PS works then the other PS might be bad or have bad connectors. 

If that doesn't work then I would also look at the power source to see if its supplying enough power.

To be specific ,generating alarm related to the power source, but problem is once we off power source A it generate power alarm related to the source A but it doesn't go once we normalize power , it remains  util remove and reinsert the module , in this time when we switched off power source B  alarm only showing for power source A.

It seems you have a faulty power source A. Do an RMA.
