I have a number of Cisco 2811 routers at various remote locations. Every one of them experiences this same issue. I have upgraded IOS twice on them to no avail.
Upon router boot, SPID1 establishes but SPID2 stays in Awaiting Establishment.
Example:

ACTIVE

TEI = 88, Ces = 1, SAPI = 0, State = MULTIPLE_FRAME_ESTABLISHED

spid1 configured, spid1 sent, spid1 valid
Endpoint ID Info: epsf = 0, usid = 0, tid = B
TEI Not Assigned, ces = 2, state = 3(await establishment)
spid2 configured, spid2 NOT sent, spid2 NOT valid
Layer 3 Status:
0 Active Layer 3 Call(s)
CCB:callid=0, sapi=0, ces=2, B-chan=0, calltype=INTERNAL (BR0/1/0) SPC C all
Active dsl 2 CCBs = 1
The Free Channel Mask: 0x80000003
If I shut the BRI interface, clear the module, and un shut it, the 2nd spid established without issue.

Any suggestions of where to look next? It doesn't matter who the ISDN carrier is, they all do this at boot.

Correction, I had to clear the interface on this particular router twice in order to establish the 2nd SPID

You are running into this bug. Workaround options are listed at the bottom.

2nd SPID always stays INVALID until sh/no shut on the bri interface
Basic Description :
-----------------
When a known working ISDN BRI line is plugged into a router, the
2nd SPID always stays INVALID as shown by "show isdn status" :
spid2 configured, spid2 NOT sent, spid2 NOT valid
As a result only one (the first) B channel can make/receive calls.
Conditions:
----------
The problem is not seen if you are running some 12.3 code instead of
12.4.
Symptoms:
--------
-Similar to CSCsa87703.
-However, the problem is seen on both the version 2 (WIC-1B-U-V2) and old
version 1 cards (NM-xB-U).
Workaround:
----------
-Configure "isdn twait-disable" under the BRI interface, or
-Enable isdn q921/q931 debugs. Enabling the isdn debugs and doing a shut/
no shut on the BRI makes the 2nd SPID goes valid (just as it was in
CSCsa87703), or
-Downgrade to 12.3
