I am facing Latency and Jitter issue on few sites. What is the best way to test those hop by hop between two sites?

We do not have enough information to be able to give good answers to your question. Can you tell us more about your network, especially about the connection from your site to these other sites? Are these hops going through the Internet? Or are they going through a private network that you control? Also what kind of network equipment do you have at each site?
If we knew these things we might be able to give you better suggestions.

Traceroute can be use to obtain (rough) hop-by-hop latencies values.
