Hi Patel
as the hosts you want to permit are indevidual hosts and not in common subnet so you have to create single statment per host
EX
ip access-list standard internet
permit host 192.168.254.100
permit host 192.168.254.101
permit host 192.168.254.102
permit host 192.168.254.103
permit host 192.168.254.104
permit host 192.168.254.105
permit host 192.168.34.100
permit host 192.168.34.101
permit host 192.168.34.102
permit host 192.168.34.103
permit host 192.168.34.104
permit host 192.168.34.105
interface Dialer1
ip access-group internet out
