The SIP is dead. Contact TAC to get the appliance RMA-ed.

Maybe the problem in software? It may help to upgrade IOS ???
What is the most stable version?

Slot 0 is your SIP. This controllers your Ethernet port. The logs you've posted shows the router is dead. Contact Cisco TAC and get the chassis RMA-ed. There's a chance the appliance is still under warranty.
