Troubleshooting command needed

We are running a new application between our head office and remote sites. This apps communicates like client/server using some specific ports. The problem is, some ware in the middle of the service providers network these ports are filtered and the traffic is not passing through and all others are working fine.
Is there any diagnostics command which will tells us in which hop the packets are getting dropped which uses that specific port.

You could try using the extended traceroute command if the ports being blocked are UDP -

Are you sure it is the SP that is filtering these ports ?. Have you talked to them as generally they do not filter customer traffic in that way.

While you're trying to get this issue resolved, if you must get the application to run NOW, you might be able to run it across your SP networks within something like a GRE tunnel.
PS:
It's odd for the provider to block ports. Are you sure there isn't a MTU issue?
