Broken flash memory for ISR4000/8000 Series Routers (SDWAN)

Has anyone come across the issue for the below:-

Broken flash memory for ISR4000/8000 Series Routers (SDWAN devices failing due to flash corruption), IOS 17.09.05a

Hardware flash failure for the SDWAN router, requiring the device to be RMA’d. The router goes into ROMMON, and unable to restart or re-install the IOS

Was the RMA requested by Cisco TAC? 

Thank you for replying, yes we have had to RMA a number of routers 

Thanks Leo for your reply, the issue was not related to this field notice and also checked agaist the serial device number check relating to this Field Notice.

Tried to execute the commands as : dir bootflash:, more bootflash:, dir flash, dir but gets the errors as “%Error opening harddisk:/ (Unknown error 144)”
